Your health insurance premiums and your dental insurance premiums will be taken out on a pre-tax basis automatically. Please review the attached document for information on Flexible Spending Accounts and you can enroll using the interactive enrollment form below. If you wish to wiave the health and/or dental pretax option, please complete the waiver on the Enrollment Form. The deadline for enrolling is November 16, 2009.
