OPTIONAL LIFE INSURANCE

 

If you are currently enrolled in optional term or universal life coverage and are eligible for a $5,000 increase in coverage amount, information will be mailed to you at your home address.  If not presently enrolled, you may be eligible to enroll in an optional life insurance plan by presenting evidence of insurability through a health questionnaire.  To apply, obtain forms from the Human Resource Office.
