TRANSFER MEDICAL PLANS
Things to think about when choosing a health plan:
*Are the physicians (primary care, pediatrician, OB/GYN, specialist), hospitals, and pharmacy we use participating in the
network?
*Revew the benefits structure in the comparison chart
*How much will I have to pay for each month as my share of the premium?
*I have a dependent/college student that lives outside of the State of Tennessee. What type of coverage is provided for
them?
What do you need to do?
*Complete the Enrollment Change Application - This must be completed and returned to the Human Resource Office by
the close of business no later than November 15, or your coverage will not be transferred.
*For HMO coverage you must complete the Enrollment Change Applicant and a Primary Care Physician Card (PCP) and
return it to the Human Resource Office. Your ID cards may not be issued nor claims praid until a selection has been
made.
