Performance Evaluation and General Instructions
You may print the Performance Review
Form and the optional Individual Assessment Plan, complete all parts/sections and
forward through normal communication channels to the Vice President
of your area by April 30, 2007.
For personnel changes occurring effective January 1, 2007,
the supervisor of the position for the January 1 - December
31, 2006 period would complete the evaluation. Personnel who
have not completed their six-month probationary period prior
to December 31, 2006 would not have the annual performance
review completed until that probationary period is completed.
