This is a checklist that you may print for your convenience.
1. Complete and submit admissions application along with a non-refundable
$10.00 application fee to Cleveland State Admissions Office.
2. Request official high school transcript or GED Scores and/or Certificate to
be mailed to the Cleveland State Admissions Office.
3. Request college transcript(s) be sent to the Cleveland State Admissions
Office.
4. Check with Cleveland State Admissions Office for information on health
immunization requirements.
