Vendor Registration Form

Attendees will be able to ask exhibitors questions on a one-on-one basis. As an
exhibitor, you will have the opportunity to share information and highlight your
program, products and/or services, while meeting and educating attendees.

Registration Deadline

March 1, 2013 (No refunds after this date.)
Any registration after March 1 will not receive all forms of recognition.

Sponsor Levels

Oak-$500–Premium location for exhibit booth, two event t-shirts, article in Green
Day insert, featured in Green Day advertisements, articles and signage.
Maple-$100–Exhibit booth, one event t-shirt, featured in Green Day article and signage.

Basic Exhibit Fee

Magnolia-$25– Exhibit space with one table and two chairs • $50– exhibit space
with one table and two chairs & electricity
